Registration for all grades - please contact the main office for the hours.
The following documentation will be required for enrollment:
- An original state birth certificate.
- All the immunizations that your child has received to date so our school nurse may review the immunizations and compare them with the newest state requirements.
- Proof of residency (deed, real estate tax bill, or any two of the following: lease with rent receipt, current utility bill, voter registration card, or other approved documentation).
- For Kindergarten students, a completed physical by your doctor, recorded on a Belmar Elementary School health form, by September of the current school year. Forms will be available at the time of registration or in the nurse's office.
If your child has not completed the required immunization schedule, you must have a provisional admittance form signed by your doctor. This form may be obtained at the nurse's office and should be returned to the school nurse.
